Data consent statement

Through completing assessment you are consenting to have your information stored confidentially on our secure database in accordance with the principles of the Data Protection Act 2018

Data may be shared with the Department of Health so that they can monitor our service activity, and which enables us to continue providing this service to our colleagues in the NHS and voluntary sector. This is anonymous so you will not be identified in any way but I need to check that you are happy for us to use your information in this way.

Please be assured that your data will not be used for any other purpose or shared with anyone outside the Here For You service at this point, although if you would like to go ahead with any treatment, we will need to share this information with the treating organisation, so that you can get the best help possible. Your therapist will discuss with you the data governance arrangements we have in place to ensure your information is stored and managed safely and securely within the relevant service.
